Constituent must appear in person (Video/conference calls are prohibited)
Constituent must provide valid government identification with photo and depending on circumstance, may need to provide a secondary proof of identification.
Satisfactory Identification
- Driver’s license or non-driver’s ID
- U.S. Passport
- U.S. Government employee’s ID, including military personnel
- State, county and local government IDs issued to employees and to licenses
- Inmate IDs issued to incarcerated prisoners
- Driver’s licenses from other states
- Canadian & Mexican driver’s licenses and another verifiable ID
- Permanent Resident Alien (Green Card Holders and another verifiable ID)
- Naturalized Citizens (Must have Naturalization Papers and another verifiable ID)
- Foreign Passports (Must have current Visa and another verifiable ID)
Note: Identification must be current AND contains, a photograph and description of the person named on it, signed by the person, has a serial number or other identifying number to verify authenticity.
- Constituent must not sign document(s) prior to notary.
- All documents must be signed in the present of the notary.
- If document(s) are signed prior to notary visit, the constituent will sign document again in the presence of notary official.
- Constituent must under oath or affirmation (or swear) under penalty of perjury that you are the person identified, and what you are about to say via documents/identification, etc., are true.
